Your personal data will be recorded and shared with our Data Governance Team and if necessary to the department that use, process, or store your personal data, or that we need to work with in order to respond to your request. If any necessary forms are required in order to release your Personal Information or those on the behalf of a requester, our Data Governance Team will reach back out with the applicable forms to complete.
